  • To fill the application, you have to make sure the information is correct, the computerized system will refuse any missing and erroneous information.
  • For those under 18 years, the ID and the signature of your parents are mandatory.
  • It is essential to inform your family about your will to donate.
  • لملء الاستمارة، يجب التأكد من صحة المعلومات، فإن نظام المعلوماتية المعتمد من الهيئة الوطنية يرفض الإستمارة التي تحتوي على معلومات مفقودة و/أو خاطئة
  • بالنسبة للّذين دون الـ 18 سنة، ان نسخة عن بطاقة هوية الأهل وتوقيعهم إلزاميّ
  • من الضروري إبلاغ عائلتك برغبتك في التبرع
